CPR/First Aid Permission Form Below!

Please complete the form to grant permission for your child to participate in the CPR/First Aid Training on Tuesday, March 17th and Tuesday, March 24th. Please note that every child must plan to attend both sessions as the training will require them to attend both dates to receive a certificate.

Paper copies of the permission form can also be found at the EMDIC and can be returned with your child.

This event is being made possible with the support of AllOne Charities.

EMDIC is excited to offer a CPR/First Aid Training and Certification class beginning Tuesday, March 17th. On Tuesday, March 17th, and Tuesday, March 24th, an instructor from the Red Cross will be at the center to provide this class. Youth who sign up must plan to attend BOTH sessions, as the training will span both dates, and are required to receive the certificate.

I give my permission for my child to participate in CPR/First Aid training with the EMDIC on Tuesday, 3/18, and Tuesday, 3/24. My child has no medical condition(s) that would prevent them from participating in CPR/First Aid Training. By providing a signature, I am acknowledging that my child plans to attend both dates for the training, which will be required to receive the certification. I understand that my child will be expected to follow the EMDIC Code of Conduct while participating.
